Jacob Leon Self, 26, of Stewart, passed away Saturday, June 5, 2021 at Ohio State University Hospital in Columbus.
Born August 23, 1994 in Athens, Ohio, he was the son of Timothy and Teresa England Self. Jake attended the Stewart Wesleyan Church, loved camping, cooking and spending time with his nephews.
He is survived by Cody McMantis; his parents, Tim and Teresa Self; two brothers, Shawn (Brittney), Isaac (Bennett); two sisters, Molly (Ryan) Chute and Olivia McCune; three nephews, Noah, Kayden and Tristan; one niece Hattie; maternal grandmother Dianne “Granny” England; paternal grandmother Sharon Spears; uncle Jason (Tabby) England; aunt Shannon Self; several aunts, uncles, cousins and a special friend Daniel Duvall
He was preceded in death by maternal grandfather Shirel England; aunt Melissa England, one cousin Braxtyn England.
Funeral services will be at 1:00 PM Wednesday June 9, 2021 at the White-Schwarzel Funeral Home in Coolville with John Mills officiating. Burial will take place in Coolville Cemetery. Visitation will take place Wednesday from 11:00 AM till 1:00 PM
